$1000 sign on bonus paid after the 6th full month of therapist being hired.  Hired date is the date all credentials are submitted and orientation has been completed.  For example, if a therapist is hired January 15th the first of 6 months will be February.  To earn $1000 bonus therapist must complete at least 100 visits in the above listed zip codes within previously listed time period and therapist must still be active.  Active status means they accepted at least 1 referral in the 6th month and have not put in resignation notice.  Bonus will be paid by the 20th day of the following month.   To be eligible therapist must have not previously worked for California Therapy Services.
